who are low income or may not have family or friends close by.
To receive a shoebox (or help someone you know receive a box),
please fill out the form and put it in the box provided

The “Shoeboxes of Joy” Holiday Project is a volunteer project and is funded by donations from the community. As such, is limited by the number of donations received. Recipient forms
should be received by Shoeboxes of Joy no later than December 15th. A request form is required for each box delivered.
